We are excited to share the news that AFS will soon launch a major initiative in San Mateo County, focusing on providing Intensive Care Coordination (ICC) services. This groundbreaking program marks a significant milestone in California’s child and youth healthcare. It is meticulously crafted to deliver extensive case management services, ensuring a holistic approach to care.

Central to our mission is the emphasis on reunification, permanency, and empowering children and families by prioritizing their voices and choices in their care journey. This initiative underscores our commitment to enhancing the well-being and future of our community’s youth.

Understanding ICC: A Comprehensive Service for Beneficiaries

Intensive Care Coordination, commonly abbreviated as ICC, is a specialized service model focusing on a trio of essential components:

  • Assessing Beneficiaries: Thorough evaluations to understand the unique needs of each child.
  • Planning Care: Developing tailored care plans that align with individual requirements.
  • Coordinating Services: Ensuring seamless health and support services integration.

Eligibility and Accessibility

The ICC program is tailored for children and youth under 21 eligible for Medi-Cal services and meets specific medical necessity criteria. Importantly, access to ICC does not require an active child welfare services case, making it readily accessible to a broader range of needy families.
