Enhanced Care Management (ECM) is a transformative program designed to bridge gaps in care and provide crucial support, ensuring families have access to the resources they need to build stable, fulfilling lives.

Launched in August 2023, ECM is already making a significant impact in Alameda County. It offers a personalized, hands-on approach to care that empowers families to regain stability and independence.

What is ECM?

ECM is a preventative and proactive care coordination program that connects individuals and families with essential support services. Whether it’s housing, food, transportation, or access to medical and mental health care, ECM acts as a safety net, ensuring that no one has to face life’s challenges alone. Rather than addressing problems in isolation, ECM emphasizes holistic solutions—helping families break cycles of crisis and strive for long-term stability.

How Does ECM Work?

The ECM process begins with referrals, often made by healthcare providers who recognize that a patient’s well-being is affected by external factors like unstable housing, food insecurity, or lack of transportation. Once a need is identified, the ECM team intervenes to offer personalized, hands-on support.

Key Steps in ECM Support:

  1. Comprehensive Risk Assessment – The ECM team evaluates housing, healthcare, income, and safety challenges, pinpointing areas of urgent need.
  2. Personalized Care Planning – A tailored plan is created to connect individuals with resources catering to their needs.
  3. Hands-on Support and Advocacy – ECM team members guide clients through application processes, coordinate services, and ensure follow-through so that no family falls through the cracks.

For instance, a client facing food insecurity might be linked with local food banks and nutritional assistance programs, while a survivor of domestic violence could receive help accessing safe housing and trauma-informed counseling.
